- Patient with end stage kidney disease on dialysis.
- Patient with advanced chronic kidney disease stage 4 or 5 with estimated gfr less than 20 mill per minute.

Types of kidney Transplants:
Living donor kidney transplant
A living donor transplant is a procedure during which a kidney is reviewed received from a healthy donor and surgically placed in an individual with kidney failure.
The living donor is often a first degree relative or a family member from an extended family such as grandmother, grandfather, parent, sibling, spouse, child, uncle, aunt & cousins.

Types of deceased donor Organ:
- Standard criteria donor: Less than 50 years of age with no comorbidities.
- Expanded criteria donor: More than 50 with high BP, CVA, or creatinine more than 1.5 mg per dl.
Double Kidney Transplant:
If both kidneys of the diseased doner are compromised then sometimes we have to remove both the kidneys for the transplatation into a single recipient.

Donor for living kidney transplant undergoes multiple level of test to being considered fit for kidney donation Three levels of test are conducted for the donor and if the donor found fit for the donation legal process starts
Recipient evaluation it includes test which are required for medical evaluation and surgery purposes patient will be scheduled to see the transplant team For the evaluation for kidney transplant
Evaluation will consist of :
- Medical evaluation
- Cardiology evaluation

Based on the physician findings during the evaluation appointments additional testing and consult may be needed.
Once the evaluation is complete and legal process is done patient will be presented at the hospital based authorization committee meeting . The committee may approve the patient for listing require additional information prior to making a decision or determine that the patient is not a candidate for transplant.
Waiting for Transplant:
Those patients who do not have appropriate donor in the family are advised to get registered themselves in the cadre registration program for kidney transplant
The patient who gets registered themselves for the cadaver registration program they are listed according to the NOTTO ID’s . Once we receive any information for brain dead organ donor the listed patient are selected as per the government protocol since we have started this program we have done two deceased organ donor kidney transplant which has led to increase in the number of patients who want cadaver organ.
Discharge Planning and Recovery:
Your length of stay in hospital will depend on few factors including type of transplant your body’s acceptance to the new organ and your overall health . Normally donor is discharged on second day of his laparoscopic donor nephrectomy and recipient is discharged on 5th day of surgery in our hospital our discharge patients are taught well how to take care of themselves, how to schedule their hospital visits and how to continue their immunosuppressive drugs . Also you are taught how to monitor your liquid intake and output our transplant team will be monitoring your health closely after the transplant and during your follow up appointments therefore it is very important to get your regular investigation done schedule follow up visits.

Tips for healthy transplant recovery

Call the transplant team if you have:
- A fever higher than 100 degree Fahrenheit
- Flu like symptoms
- New pain around the transplant area
- Swelling due to fluid retention
- Sudden weight gain more than 4 pound in 24 hours
- Significant decrease in appetite or urine output
What you need to know about rejection?
Any of symptoms mentioned above could be a sign of graft rejection one of the most common complication after kidney transplant

- Renal function test
- Renal flow scan (Doppler study)
- Kidney biopsy
What is a kidney biopsy?
It is a procedure Mostly done on OPD basis where A small piece of new kidney is removed with the needle and send for testing you are discharged the same day following surgery and report follows also 7 to 8 days later . you need to consult the transplant team if the biopsy confirms rejection because it needs immediate treatment.
